Detailed Overview

Coordinates, screens and prioritizes hospice palliative client activities by assessing clients, consulting with the interdisciplinary team and coordinating client care across the continuum of care; acts as a clinical resource to other nurses and team members by providing clinical guidance and/or nursing consultation; intervenes in client care provided by other nurses and/or healthcare providers, as required to ensure that client outcomes align with care plans;

implements, evaluates and recommends clinical education plans for nurses and other staff; provides nursing and clinical education/training through individual and/or group assessment of clinical skills; reviews and evaluates care plans developed for clients and takes corrective action.

Responsibilities

  • Supports the interdisciplinary team by monitoring the standards of care and acting as a clinical nurse resource; assesses, demonstrates and directs the provision of care for hospice palliative clients; role models, coaches and mentors the team in therapeutic communication with clients, families and caregivers.
  • Coordinates and prioritizes client care activities by providing formal clinical consultations to nurses, interdisciplinary team members and students; reviews client needs and progress, troubleshoots and provides clinical guidance to adapt care plans; collaborates with staff on referral options to other healthcare professionals.
  • Consults with the interdisciplinary team and other healthcare providers to resolve client care issues; supports staff with critical thinking to address variances from guidelines; works with client, family and caregiver to understand and minimize variances.
  • Develops and implements clinical education, training and orientation in the designated clinical area for the interdisciplinary team; assesses learning needs and updates learning plans; delivers teaching/learning strategies aligned with outcomes.
  • Assesses clinical skills by reviewing competencies and quality indicators; designs and evaluates clinical education programs in hospice palliative care; provides training to new team members and ongoing continuing education.
  • Acts as preceptor, clinical resource and mentor; coordinates with the Manager on skill deficiencies; provides instruction to staff and ensures learning outcomes are met; promotes best client practices.
  • Coordinates quality improvement and risk management activities; evaluates changes to care practice, completes client incident reports, conducts safety audits and makes recommendations to update policies and procedures.
  • Participates in interdisciplinary rounds and care conferences; advocates for the client’s dignity, autonomy, and access to information.
  • Participates in staff recruitment and performance evaluations by contributing to interviews and providing feedback to management on selection and orientation needs.
  • Promotes research in nursing by integrating evidence-based findings into practice; supports clinical practice with current research.
