Queens Park Care Centre (QPCC) is located in New Westminster and provides rehabilitation services, an adult day program and a long‑term care community. QPCC has an outpatient rehab service and 76 specialized inpatient rehabilitation beds. These provide intensive rehab for different patient populations including Brain Injury, Spinal Cord Injury and Stroke rehab patients as well as lower leg amputees, complex neurological conditions and other muscular skeletal injuries.

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person‑centered care, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in an acute rehabilitation unit.
Responsibilities- Assess patients within an acute rehabilitation setting and collect information from a variety of sources using observation, communication, data review and physical assessment. Formulate a nursing diagnosis and determine whether the condition can be improved or resolved by an appropriate nursing intervention. Plan, perform and evaluate nursing interventions and care.
- Collaborate with patients and their families/caregivers in developing the care plan and encourage family participation in the development, implementation and ongoing modification of the care plan. Establish a therapeutic relationship by demonstrating empathy, trust and respect, advocating for the patient's rights to privacy, dignity and access to information, and ensuring the patient's choice and autonomy in decision making and care planning.
- Participate in the care and discharge planning process for patients on admission by ensuring the care plan is established and implemented effectively, updating the care plan and intervening as appropriate, collaborating with the interprofessional care team to ensure comprehensive care planning and delivery, and coordinating with the Patient Care Coordinator on issues that will affect the care/discharge plan.
- Coordinate nursing care for the patient; set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols and practice standards.
