Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Registered Nurse provides direct patient care in acute inpatient areas, working collaboratively and in partnership with the interprofessional care team, offering person‑centered care.

Responsibilities
  • Assess patients and collect information from a variety of sources using skills of observation, communication, data review and physical assessment. Make a nursing diagnosis and determine whether the condition can be improved or resolved by appropriate nursing intervention.
  • Collaborate with patients and their families/caregivers in developing the care plan, encouraging family participation and establishing a therapeutic relationship characterized by empathy, trust, and respect.
  • Participate in care and discharge planning processes, ensuring the care plan is established and implemented effectively and updating it as appropriate.
  • Coordinate nursing care for the patient, set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols and standards.
  • Collaborate with interprofessional team members—including physicians, licensed practical nurses, patient care coordinators, clinical nurse educators, and other healthcare professionals—to identify and resolve issues and adjust care plans as needed.
  • Teach patients and families about illness prevention, health restoration and maintenance, assessing learning needs and providing necessary information and demonstrations.
  • Plan and discuss referral information with patients and families, initiating referrals to appropriate health care and community agencies.
  • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes, and referrals in accordance with practice standards and Fraser Health procedures.
  • Participate in nursing committees, care conferences, and councils as assigned or agreed with the manager.
  • Contribute to research and quality improvement activities by identifying patient care issues and recommending corrective actions or changes to practice.
  • Attend in‑services and other educational programs, identify personal learning goals, and maintain current clinical competence within the area of practice.
  • Act as a preceptor or mentor for newly assigned nurses and students, providing guidance and training during orientation.
  • Provide feedback to the manager regarding the effectiveness of patient care programs, policy/clinical decision‑support tools, and equipment.
  • Perform other related duties as required.
  • Qualifications

    Education and Experience

    Graduation from an approved School of Nursing and current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
