This position at Vancouver Coastal Health focuses on the Seniors Remote Monitoring Pilot. You will collaborate with a multidisciplinary team, ensuring client-centered care in compliance with BCCNM standards. Engaging effectively with clients, you’ll perform intake functions that guide them to suitable health resources while maintaining accurate records of assessments and referrals.

Key Responsibilities:

• Perform initial screenings and triage for client requests

• Facilitate client access to appropriate community resources

• Provide informative consultations to clients and families

• Document and maintain comprehensive client records

• Engage in quality improvement efforts and team discussions
Requirements:

• Registration with BCCNM as a Registered Nurse

• Three years of relevant nursing experience

• Strong understanding of home health care regulations

• Ability to provide trauma-informed, culturally safe patient care

• Proficient in communication and teamwork
