Salary Details

The salary range for this position is CAD $41.42/Hr.

Job Summary

This intake position supports the Seniors Remote Monitoring (SRM) Pilot, a regional initiative aimed at supporting the older adult population to live safely in the community with additional monitoring resources and care coordination with home health. While the intake nurse role is based out of the Banfield Transitional Care Unit, it is designed to support the broader regional implementation of the pilot.

Within the context of a client centred framework and continuum based care model and in accordance with British Columbia College of Nurses and Midwives (BCCNM) for a Registered Nurse, standards of professional practice, Code of Ethics, and the vision and values of the organization, including Indigenous Cultural Safety, cultural humility and anti‑racism principles, works as a member of an interdisciplinary team to conduct the centralized intake functions for the designated site/program services to ensure linkages between client and the appropriate resources occurs.

Performs initial screening, triage and referrals based on client needs, urgency of request and best practices to determine the appropriate treatment and educates and provides consultation to client/family. Provides information to health providers and the general public, establishes and enhances relationships/communication within the designated program services and other community resources, prepares and maintains client records, maintains and provides accurate intake/referral data and statistics, participates in team meetings and continuing education, and engages in on‑going team planning and quality improvement.

Qualifications

Education & Experience

  • Current practising registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years recent related experience in the designated clinical program area or an equivalent combination of education, training and experience.

Knowledge, Skills & Abilities

  • Broad knowledge of nursing theory and practice within a client and family centred care model.
  • Broad knowledge of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for nursing.
  • Demonstrated ability to provide client‑focused, trauma informed, harm reduction, and culturally safe care in completing initial client assessments and interventions.
  • Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
  • Knowledge of community resources available for client/family support and related health services.
  • Broad knowledge of other health care disciplines and their role in client care.
  • Broad knowledge of adult education principles, methods and tools.
  • Demonstrated ability to problem solve using sound judgement in applying critical thinking skills within safe limits of client care.
  • Demonstrated ability to communicate effectively, both orally and in writing.
  • Demonstrated listening and information seeking skills that promote communication and lead to a co‑operative approach to problem solving within a multidisciplinary setting.
