This position at Royal Columbian Hospital in New Westminster, B.C. offers the opportunity to consolidate your nursing skills by rotating through specialty nursing units prior to and/or during postgraduate coursework.

Position Overview

Provide direct patient care to cardiac patients while completing applicable postgraduate specialty nursing courses and programs within agreed time frames. Work collaboratively with the interprofessional team to deliver person‑centered care following British Columbia College of Nurses and Midwives standards.

Responsibilities
  • Assess patients and collect information from a variety of sources using observation, communication, data review, and physical assessment. Make a nursing diagnosis of the patient's condition and determine whether the condition can be improved or resolved by an appropriate nursing intervention. Plan, perform, and evaluate nursing interventions and care.
  • Collaborate with patients and their families/caregivers in developing the care plan and encourage family participation in the development, implementation, and ongoing modification of the care plan. Establish a therapeutic relationship by demonstrating empathy, trust and respect, advocating for the patient's rights to privacy, dignity, and access to information, and ensuring the patient's choice and autonomy in decision‑making and care planning.
  • Participate in the care and discharge planning process for patients on admission, ensuring the care plan is established, implemented, and evaluated effectively. Update the care plan in a timely manner, intervene as appropriate, and collaborate with the interprofessional care team to ensure comprehensive care planning and delivery. Communicate with the Patient Care Coordinator on issues that will affect the care/discharge plan.
  • Coordinate nursing care for the patient; set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols and practice standards.
  • Collaborate with members of the interprofessional care team, including Physicians, Licensed Practical Nurses, Patient Care Coordinator, Clinical Nurse Educators, and other healthcare professionals/providers in identifying and resolving issues, adjusting care plans as required, and responding to changes in patient condition.
  • Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to help patients accomplish their health‑care goals. Assess learning needs and provide information and demonstrations of care activities to support self‑care and the discharge plan.
  • Plan and discuss referral information with patient and their families/caregivers and, in collaboration with the interprofessional care team, initiate referrals to appropriate health‑care/community agencies.
  • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
  • Participate on nursing committees, care conferences, and councils as assigned or as agreed to with the Manager.
  • Participate in research and quality‑improvement activities by identifying patient‑care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
  • Participate in and attend in‑services and other educational programs. Identify own learning goals and maintain and update current clinical competence and knowledge within area of practice.
  • Provide feedback to the Manager/delegate regarding the effectiveness of patient‑care programs, policy/clinical decision‑support tools (e.g., clinical pathways, standards, order sets, and procedures), and equipment.

Qualifications
  • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
