Queens Park Care Centre (QPCC) is located in New Westminster and provides rehabilitation services, an adult day program and a long‑term care community. QPCC has an outpatient rehab service and 76 specialized inpatient rehabilitation beds. These provide intensive rehab for different patient populations including Brain Injury, Spinal Cord Injury, Stroke rehabilitation patients, as well as lower leg amputees, complex neurological conditions and other musculoskeletal injuries.

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Nurse Clinician utilizes evidence‑informed practice to provide consultation and clinical expertise in skin and wound care and prevention that will achieve the most effective outcomes. The role includes direct care skills modeling, complex wound modalities, and education of staff and patients/families.
Participation in research, policy, quality improvement, and risk management activities is also required.
- Demonstrate expertise and provide clinical leadership in evidence‑informed practice related to skin and wound care; review literature, consult with experts, collaborate with interprofessional team members, evaluate clinical practice, recommend changes to existing standards, and participate on regional and provincial committees.
- Model direct patient care skills to staff and students in skin and wound care, and provide just‑in‑time education at the point of care.
- Perform identified complex wound modalities as needed and follow up on complex skin and wound cases.
- Determine education requirements that meet clinical and learner needs, in collaboration with the Manager/Director/Clinical Nurse Educator/Clinical Nurse Specialist; complete learning needs assessments of individuals or groups.
- Develop and revise teaching plans based on identified learner needs, chart audits, patient outcomes, and introduction of new skills/procedures.
- Deliver learner‑centered education to individuals or groups of staff and patient/families through presentations, facilitated groups, and lectures.
- Evaluate effectiveness of education programs and consultations to ensure skin and wound care objectives/learning needs are met and identify opportunities for quality improvement.
- Plan and schedule education activities and maintain a system for tracking attendance; keep statistics for reporting and quality improvement purposes.
- Collaborate with other practice leaders to provide consultation and performance support to clinical staff, students, and staff nurses regarding skin and wound care.
- Support staff in planning and delivering patient/family education about discharge planning, illness prevention, and health maintenance.
- Participate in quality improvement and risk management activities, evaluating effects of changes in practice, making recommendations for improvement, and conducting audits.
