Position: Registered Nurse, Cardiac Access & Assessment - Triage - Royal Columbian Hospital (New Westminster)
Location: new westminster

Overview

Salary range: CAD $41.42 - $59.52 / hour.

Royal Columbian Hospital is located adjacent to the Sapperton Sky Train station on the Expo Line. The hospital Redevelopment Project (RCHRP) is a multi-year, multi-phase effort to increase capacity, beds and services.

Responsibilities
  • Performs direct patient case management including:
    • Pre-admission screening of referred patients by interviewing patients, reviewing histories, conducting nursing assessments, identifying potential problems, and coordinating with physicians to optimize conditions and ensure required diagnostic tests are booked and completed.
    • Full nursing review including assessment of diagnostic testing and reporting abnormalities to physicians as appropriate.
    • Provide patient/family-centered education in person and virtually; ensure information is timely and culturally appropriate, including translation services as needed; serve as point of contact and provide ongoing telephone support to wait-list patients and families.
    • Conduct nursing assessment and patient teaching in person and via virtual strategies.
  • Performs indirect patient case management including:
    • Communicating potential problems with clinical areas, physicians and offices as required.
    • Participating in daily patient reviews and triage to align procedure slate with patient demand and acuity; ensure patients are ready and able to attend procedures.
    • Attending weekly Heart Team rounds when required; provide information and assessment findings on high-risk patients to support collaborative decision making.
    • Share expertise with other health care professionals.
  • Performs triage activities including:
    • Notifying cardiac and referring physicians of changes in patients’ conditions; coordinating with physicians to develop alternatives to meet waiting time guidelines.
    • Providing relevant information to clinical partners to facilitate decision-making and timely access to care.
    • Tracking patients awaiting cardiac procedures/interventions and monitoring wait times and patient condition.
  • Performs administrative duties including:
    • Data entry for Fraser Health and Cardiac Services BC database systems.
    • Assisting the coordinator with timely updates and recording in relevant databases.
    • Assisting the Cardiovascular Coordinator to integrate tracking of quality indicators and outcomes; contributing to quality improvement projects.
  • Performs other related duties as required.
  • Qualifications

    Education, Training and Experience

    Graduation from a recognized nursing program. Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Two (2) years’ recent related clinical experience in the designated clinical area, or an equivalent combination of education, training and experience.

    • Clinical proficiency in the designated clinical nursing area.
    • Ability to complete initial and ongoing patient assessments and provide nursing care with appropriate interventions.
    • Effective communication with patients, families/caregivers, the public, medical staff and the interprofessional care team using verbal, written and electronic means.
    • Ability to promote person-centered care, empathy, cultural sensitivity, patient advocacy.
    • Ability to integrate and evaluate data from multiple sources to problem-solve.
    • Ability to teach and instruct patients and families about essential health topics.
    • Ability to organize and prioritize work.
    • Ability to collaborate with the interprofessional care team.
    • Ability to use evidence-based information to support nursing decisions.
    • Ability to operate related equipment including computer applications.
    • Physical ability to perform the duties of the position.
