This Casual position involves working independently and collaboratively within an interprofessional team. You'll manage client assessments, interventions, and care planning, emphasizing health restoration and chronic disease treatment.
Your role is critical in supporting clients and their families within the community.
Key Responsibilities:
• Establish therapeutic relationships with clients and families
• Screen referrals and conduct individualized assessments
• Develop comprehensive client care plans
• Provide direct clinical care and education
• Facilitate interprofessional care conferences
Requirements:
• Current registration with BCCNM as a Registered Nurse
• One year of clinical experience with chronic conditions
• Valid BC Driver's license and personal vehicle access
• Strong assessment and care planning skills
• Ability to work independently and within a team
