We are looking for an experienced Licensed Practical Nurse committed to delivering goal-directed nursing care in accordance with provincial standards. This position entails collaboration with an inter-professional team under the guidance of the Director of Care, ensuring comprehensive care for our residents. Your responsibilities will include clinical assessments, documentation, and direct patient care tailored to individual needs.

Key Responsibilities:

• Complete and manage residents' clinical care assessments

• Evaluate care plans and modify as needed for effectiveness

• Assist with scheduling care conferences for residents

• Deliver medication and treatments in compliance with standards

• Partner with the Director for smooth resident transitions

Requirements:

• Diploma in Practical Nursing and active registration

• At least 3 years in seniors' healthcare

• Current CPR certification

• Proficient with Microsoft Office

• Strong mentorship and leadership skills
