Make a significant impact as a Community Health Nurse with Fraser Health in Burnaby, B.C. This Casual position focuses on enhancing the health of clients through direct care and education.
As a Registered Nurse, you will work autonomously and as part of a larger interprofessional team.
Your role includes managing client populations, conducting assessments, and implementing care plans to aid in chronic disease management and client health restoration.
Key Responsibilities:
• Establish and maintain client relationships for optimal care
• Conduct thorough assessments and health interventions
• Develop and execute client-centered care plans
• Collaborate in multidisciplinary care conferences
• Maintain clinical documentation as per policies
Requirements:
• BCCNM registration as a practicing Registered Nurse
• At least one year of experience in a relevant setting
• Valid BC Driver's license and vehicle access
• Skills in client assessment and health promotion
• Ability to work in a fast-paced, team environment
