As a Licensed Practical Nurse at Age Care, you will play a vital role in delivering personalized, resident-focused care. Reporting to the Director of Care, you'll collaborate with a dedicated team, conducting clinical assessments and ensuring that care plans are accurately maintained and executed. This role also involves engaging with residents during care conferences and facilitating smooth transitions.

Key Responsibilities:

• Administer and document comprehensive clinical assessments

• Implement care plans and assess their effectiveness

• Schedule and attend care conferences with residents

• Provide direct care and medication administration

• Work closely with the Director to manage resident relocations

Requirements:

• Valid Practical Nurse Diploma and registration in good standing

• 3+ years experience in seniors' health

• Current CPR training

• Computer skills in Microsoft Office

• Leadership, coaching, and teamwork abilities
