In accordance with the competency guidelines and full scope of practice outlined by the British Columbia College of Nurses and Midwives (BCCNM) and the Mission and Values of Fraser Health, the Community Health Nurse (CHN) – Licensed Practical Nurse works independently in the community setting. The position is a relief role available until December 2026 or until the return of the incumbent.

- Establish a therapeutic relationship with clients through interpersonal and interviewing techniques, in person or over the telephone, ensuring client choice and autonomy in decision‑making and care planning.
- Screen referrals and provide individualized client assessments, interprofessional care planning, and interventions. Facilitate timely health services and coordinate the integration of care and services.
- Initiate and develop a comprehensive shared client care plan in collaboration with the interprofessional team, primary care provider, and client/family.
- Provide direct client care and identify additional care services in accordance with applicable guidelines, policies and evidence‑based best practice.
- Assign direct client care tasks to Community Health Workers as required; provide in‑home demonstrations and training; monitor delegated duties in collaboration with the Community Health Nurse – Registered Nurse.
- Facilitate care conferences to review client care plans, determine timing and referral to other services, and evaluate care effectiveness.
- Discuss clinical findings with client/family and develop action plans to improve health outcomes. Access system information and resources to incorporate findings into the care plan.
- Support clients and families before, during and after interprofessional conferences and clinic visits; advocate for client choices and needs.
- Arrange and participate in joint home visits with other healthcare professionals; provide advice on community resources.
- Provide health counselling, education, self‑management and wellness promotion through clinic, telephone or home visits.
- Maintain clinical records—intake screens, client assessments, clinical interventions, care plans and progress notes—and statistical information in accordance with established policies.
- Maintain professional practice growth by reviewing literature, attending workshops, and networking with other health professionals.
- Participate in department quality improvement and risk management activities; identify patient care issues and collect data.
- Assist in the orientation and ongoing education of nursing staff and students; act as a preceptor when appropriate.
- Provide input in developing and revising standards of care, policies and procedures; advocate for improvements in clinical practice.
