Community Health Nurse

Join Fraser Health’s dedicated Home Health team as a Community Health Nurse in Burnaby, B.C. This full-time relief role focuses on patient-centered care, chronic disease management, and supporting clients at home.

As a Registered Nurse, you will work independently while collaborating with an interprofessional team to manage client caseloads effectively. You will conduct assessments, develop individualized care plans, and support clients transitioning through various healthcare services. Your expertise in geriatric and chronic health conditions will make a significant impact in helping clients achieve optimal health and well-being.

Key Responsibilities:

• Establish therapeutic relationships with clients through effective communication

• Conduct comprehensive client assessments and develop care plans

• Coordinate health services and resources for client needs

• Facilitate client transitions across healthcare settings

• Provide health education and wellness promotion to clients

Requirements:

• Current registration as a Registered Nurse with BCCNM

• One year of recent relevant clinical experience

• Valid BC Driver's License and access to a vehicle

• Knowledge of chronic illness management and gerontology

• Ability to work independently and in teams
