In this integral role, you will report to the Primary Health Care Manager and bring your nursing experience to a community setting. The position emphasizes leadership, critical thinking, and facilitation skills to ensure quality patient care. Candidates must possess a Nursing degree and be prepared for community engagement.
Key Responsibilities:
• Coordinate comprehensive nursing services across PHC sites
• Promote health care outcomes through established practices
• Involve in community health initiatives
• Participate in evaluating health care initiatives in the area
• Deliver chronic disease management support
Requirements:
• Bachelor’s Degree or Diploma in Nursing or similar
• Minimum 2 years of related nursing experience
• Basic Life Support Certificate (BLS) required
• Facilitation and critical thinking competencies
• Knowledge of motivational interviewing beneficial
