Join the dynamic Home Health Team at Burnaby Hospital as a Community Health Nurse - Licensed Practical Nurse. In this role, you will assess and manage clients within their homes, facilitating care transitions to promote health and independence. You will collaborate with interdisciplinary teams to plan care and ensure smooth transitions for patients across healthcare settings.
Key Responsibilities:
• Conduct client assessments and develop care plans
• Provide therapeutic interventions for chronic conditions
• Facilitate care transitions between services

• Maintain clinical records and documentation
Requirements:
• Licensed Practical Nurse registration in BC
• One year of clinical experience with complex patients
• Valid BC Driver's license required
