Become a vital part of Fraser Health as a Community Health Nurse, serving Burnaby, B.C. This full-time relief position emphasizes collaborative healthcare and effective patient support.
In this Registered Nurse role, you will navigate complex client caseloads, conduct assessments, and foster collaborative care planning. Your focus on clients with chronic conditions will support their independence and well-being will also work with an interprofessional team to facilitate continuous care and transitions across various healthcare services.
Key Responsibilities:
• Initiate therapeutic relationships with clients for effective care
• Conduct thorough assessments and develop shared care plans
• Coordinate and integrate care services across healthcare settings
• Offer educational resources and support for clients' health
• Maintain accurate clinical documentation as required
Requirements:
• BCCNM registration as a Registered Nurse
• At least one year of relevant nursing experience
• Must possess BC Driver's license and vehicle access
• Comprehensive knowledge of chronic illness and gerontology
• Ability to work autonomously and collaboratively
