Position: Social Worker Role with Island Health
Location: New Westminster

Enhance patient care as a Social Worker with the Vancouver Island Health Authority. Provide essential support through assessment and intervention for clients facing barriers.

As part of a multidisciplinary team, you will contribute to client-centered care efforts that ensure optimal health outcomes. This role may involve travel and requires strong interpersonal skills to effectively coordinate care and communicate with clients and their families.

Key Responsibilities:

• Assess client needs to develop intervention strategies

• Provide referrals to community-based services for clients

• Collaborate effectively with a team of health providers

• Foster positive relationships with patients and staff

• Manage workload through effective organization

Requirements:

• Bachelor of Social Work degree is essential

• Active registration with BC College of Social Workers

• Two years of relevant experience preferred

• Valid BC Driver's License is necessary

• Training in brief crisis counseling is an asset
