As a member of the interdisciplinary team, your objective will be to promote self-determination while conducting thorough assessments and determining care plans. Your collaboration with community services will ensure that clients receive cohesive and effective care throughout the healthcare continuum.
Key Responsibilities:
• Perform in-depth psychosocial assessments for effective treatment
• Collaborate with clients and families for care plan development
• Act as a resource for community service referrals
• Maintain patient records per established standards
• Engage in community planning to enhance service delivery
Requirements:
• Current registration from the BC College of Social Workers
• At least two years' experience in a clinical environment
• Valid BC Driver's License might be necessary
• Knowledge of First Nations health issues
• Strong communication and collaboration skills
