The Clinical Triage Hub is central to ensuring patients receive timely, safe and effective care across the community. As a Senior Nurse, you will play a role in assessing and prioritising referrals, supporting the coordination of community nursing services. Using your District Nursing experience, you will make autonomous clinical decisions that help ensure patients receive the right care, at the right time.

Working closely with the Clinical Team Leader and multidisciplinary partners, you will contribute to service development. You will also mentor students and new staff, act as a role model to promote excellence in patient care.

We are seeking an enthusiastic and compassionate nurse with excellent communication, organisational and clinical assessment skills who is committed to delivering high-quality, patient-centred care. The role requires flexibility to work across a seven-day service, including rotational day and evening shifts, weekends and Bank Holidays.
