Overview

Burnsville, MN, US Full Time 1207 Assertive Community Treatment (ACT) Case Manager - Dakota County.

Location:

Burnsville. Salary Range: $58,656.00 - $66,456.00 annually. ACT is a program that functions as a hospital without walls. The ACT team provides services and supports to individuals most affected by mental illness who would otherwise be in institutional care. The Case Manager helps stabilize individuals, support recovery, and prevent hospitalization through collaborative, community-based care.

Minimum

Education and Experience:

Baccalaureate degree in social work, psychology or a related field. Two years of case management experience, including at least one year in community-based mental health with adults recovering from mental illness, or an equivalent combination of experience and training.

Responsibilities
  • Work as part of a team to develop an individualized plan of services and supports (Individual Treatment Plan) to enable each person to live as independently as possible in the community.
  • Provide services that may include skill development in budgeting, housing, transportation, hygiene, relationships, symptom management, medication adherence, and obtaining or maintaining benefits.
  • Maintain awareness of each individual’s medication adherence plan, evaluate adequacy, report problems, and recommend modifications as needed.
  • Provide collaboration and education to significant others and family members as requested to support recovery.
  • Advocate for clients to protect rights and privacy; ensure understanding of complaint and grievance procedures.
  • Treat individuals with dignity and respect, ensuring services are culturally appropriate.
  • Communicate changes in the individual’s status or needs promptly to the team.
  • Ensure services are delivered as planned in the Individual Treatment Plan.
  • Provide coverage in illness or crisis to maintain continuity of care for each person served.
  • Participate in scheduled staff meetings and trainings; maintain confidentiality and privacy per MHR policies.
  • Ensure timely completion of MHR documentation and adherence to policies and procedures.
  • Follow mandated civil commitment procedures and timelines as required by law.
  • Maintain productivity standards (e.g., outreach, caseload, contact frequency) as outlined by the Team Leader; keep chart information up to date (ROIs, profile information, status changes, contacts).
