Your Role – What You’ll Do
  • Be fully responsible for on-call coverage during your 7 days on (nights, days, weekends)
  • Respond to urgent patient needs including pain, symptom management, and crisis situations
  • Provide telephonic support to families and caregivers with empathy and clarity
  • Conduct in-home visits as needed across our 21-county service area
  • Coordinate with the IDG to ensure appropriate follow-up and care planning
  • Maintain accurate, timely documentation in our electronic health record
Qualifications – What You Bring
  • Current MN Registered Nurse license
  • Previous hospice, oncology, or palliative care experience required
  • Clinical confidence to triage and respond to a wide range of scenarios
  • Excellent communication and critical thinking skills
  • Ability to self-manage a demanding on-call schedule with professionalism
  • Reliable vehicle, valid driver’s license, and comfort driving to homes throughout the metro
  • Personal cell phone for work communication (
    device not provided; reimbursement available
    )
