Key Responsibilities

• Assess residents' physical, emotional, and psychosocial needs and develop individualized care plans

• Administer medications, treatments, and IV therapies per physician orders

• Monitor resident conditions and respond to changes in status

• Coordinate care with physicians, therapists, and interdisciplinary team members

• Supervise and delegate tasks to LPNs and CNAs in accordance with scope of practice

• Perform wound care, enteral feeding management, and infection control procedures

• Complete accurate and timely documentation in the electronic medical record (EMR)

• Participate in care conferences, discharge planning, and resident/family education

• Ensure compliance with state, federal, and facility regulations (CMS, HIPAA)

• Respond to emergencies and provide life-saving interventions as needed

• Active Registered Nurse (RN) license in the state of practice

• BLS certification required (ACLS preferred, facility dependent)

• Previous LTC or SNF experience preferred (new grads may be considered)

• Strong assessment, critical thinking, and organizational skills

• Medication management and IV therapy

• Wound care and chronic disease management

• Leadership and staff supervision

• Effective communication and documentation

• Compassionate, resident-centered care

• Long-Term Care or Skilled Nursing Facility

• May include rotating shifts, weekends, and holidays depending on facility needs
