Key Responsibilities
Assess residents' physical, emotional, and psychosocial needs and develop individualized care plans
Administer medications, treatments, and IV therapies per physician orders
Monitor resident conditions and respond to changes in status
Coordinate care with physicians, therapists, and interdisciplinary team members
Supervise and delegate tasks to LPNs and CNAs in accordance with scope of practice
Perform wound care, enteral feeding management, and infection control procedures
Complete accurate and timely documentation in the electronic medical record (EMR)
Participate in care conferences, discharge planning, and resident/family education
Ensure compliance with state, federal, and facility regulations (CMS, HIPAA)
Respond to emergencies and provide life-saving interventions as needed
Active Registered Nurse (RN) license in the state of practice
BLS certification required (ACLS preferred, facility dependent)
Previous LTC or SNF experience preferred (new grads may be considered)
Strong assessment, critical thinking, and organizational skills
Medication management and IV therapy
Wound care and chronic disease management
Leadership and staff supervision
Effective communication and documentation
Compassionate, resident-centered care
Long-Term Care or Skilled Nursing Facility
May include rotating shifts, weekends, and holidays depending on facility needs
