Staffordshire County Council’s Provider Services team is seeking Support Facilitators to deliver person-centred reablement care in the community. You will be part of an empowering service that supports individuals to thrive in their own homes.
Hours are 7:00 AM to 10:00 PM, with shifts including weekends. Applicants must live within 30 minutes of DE14 1AH
and have a valid UK driving licence with access to a vehicle to cover North/East Staffordshire. DBS check and health assessment are required.
