Overview
The Advanced Nurse Practitioner (ANP) will work autonomously within their agreed scope of practice to provide high-quality, patient-centred clinical care in a general practice setting. The post holder will assess, diagnose, treat, and manage patients presenting with acute, urgent, and chronic conditions, ensuring safe clinical decision-making, appropriate escalation, and robust documentation.
Responsibilities- Independent patient assessment, diagnosis, and treatment
- Management of acute, urgent, and on-the-day presentations
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment
- Chronic disease management
- Prescribing in line with qualification and local guidelines
- Appropriate referral, escalation, and safety-netting
- Accurate and timely clinical documentation
- Working collaboratively with GPs, nurses, pharmacists, and the wider MDT
- On occasion, carry out other tasks as workload and staffing levels permit
- Maintain accurate clinical records in conjunction with extant legislation and ensure SNOMED CT codes are used effectively
- Engage in robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance
- Professional Practice and Governance:
Work within professional boundaries, the NMC Code, and practice policies; maintain a clear understanding of own scope of practice and seek supervision when appropriate - Participate in clinical supervision, peer review, and appraisal
- Engage in clinical audit, quality improvement, and service development
- Contribute to safe systems of work, including incident reporting and learning
- Maintain confidentiality and comply with information governance requirements
- Team Working and Communication:
Work collaboratively with GPs, nurses, pharmacists, paramedics, and the wider practice team; communicate effectively with patients, carers, and colleagues - Support a positive team culture and contribute to staff wellbeing
- Participate in practice meetings and training as required
- Education and Development:
Maintain ongoing CPD in line with NMC and professional requirements; keep up to date with clinical guidelines and best practice - Support the development of other staff where appropriate (e.g. mentoring, teaching)
- Responsibilities for
Risk Management:
Identify and manage clinical risk appropriately; escalate complex or high-risk cases promptly; ensure patient safety is central to all decision-making - Comply with safeguarding responsibilities for adults and children
