Job Overview

Bury Town Integrated Neighbourhood Team has had a significant investment, which has led to the development of multiple new roles within the team. This opportunity is to join the Community Nursing Teams based in Bury Town as a Band 7 District Nurse, with a total of 45 hours per week. Both part-time and full-time working are considered.

Working alongside two other District Nurses, you will be based within an integrated community team that includes Occupational Therapists, Physiotherapists and a range of support staff. The role is a front line position delivering medical care to patients in their own homes, with excellent support from the Integrated Community Team.

To be considered, you must hold a specialised District Nursing degree following your Adult Nursing qualification. NHS work experience is essential, and Community Nursing experience is desirable.

Job Responsibilities
  • Lead and manage the District Nursing Service in the locality, providing specialist clinical and supervisory leadership to junior staff and students.
  • Manage a caseload of patients with multiple and complex needs, using evidence‑based practice to assess, plan, implement and evaluate interventions, keeping the patient at the centre.
  • Provide the highest standard of clinical care by using advanced skills and expert knowledge to holistically assess needs and deliver clinical treatments.
  • Lead service development of integrated services in partnership with peers and colleagues.
  • Use new knowledge in innovative ways and take responsibility for developing and changing practice in complex and unpredictable environments.
  • Support staff to feel confident and competent to move across organisational and professional boundaries.
  • Monitor the quality of care provision, identify and promote areas for service development.
  • Build strong relationships with partner organisations, ensuring effective flow of patient information for shared care.
Education & Qualification
  • Registered Nurse Level 1
  • District Nurse Qualification
  • Current NMC registration
  • Evidence of continuing professional development
  • Teaching, learning and assessing qualification
  • Tissue viability or leg ulcer management training
  • Teaching, learning and assessing, Level 7 or PGCE
Experience & Knowledge
  • Experience of leading a District Nursing team
  • Management of complex patients
  • Responsible for management of complex cases, sharing knowledge and skills with locality and community nurses to foster growth
  • Advanced clinical assessment skills, including history taking and physical examination
  • Experience of working autonomously
  • Experience of working with people with long-term conditions
  • Awareness and understanding of local and national priorities for patients with long-term conditions, nursing and integration
  • Ability to direct and coordinate programmes of care working collaboratively and autonomously
  • Management of a community team
  • Experience of interview and recruitment skills
  • Understanding and knowledge of the Health and Social Care Act
