Practice Nurse

This is a one-year fixed-term practice nurse post at Uplands Medical Practice, working three days a week within the nursing team of a GP practice. The core of the work is nursing care across chronic disease reviews, immunisations, cervical screening and preventative care. The practice is particularly keen on experience managing COPD, asthma and diabetes, with an emphasis on helping patients self-manage long-term conditions.

Fit depends on current RGN registration with the NMC.

• Deliver practice nurse services within agreed competencies and NMC standards
• Undertake chronic disease reviews for diabetes, asthma and COPD
• Conduct annual reviews and care planning for patients with long-term conditions
• Administer childhood, adult and seasonal vaccinations in line with national guidance
• Undertake cervical cytology screening
• Provide lifestyle advice on smoking cessation, weight management and healthy living
• Carry out blood pressure monitoring, ECGs, venepuncture and other nursing procedures as required
• Support infection prevention and control processes
• Maintain accurate patient records on EMIS Web
• Participate in clinical audits and quality improvement initiatives
• Support the achievement of QOF targets
• Work collaboratively with GPs, HCAs, pharmacists and the wider multidisciplinary team

• Registered General Nurse (RGN) qualification
• Evidence of ongoing professional development

• Current NMC registration

• Independent Prescriber Qualification
• Chronic Disease Management qualification
• Asthma Diploma or equivalent
• Diabetes Diploma or equivalent
• Paediatric Phlebotomy
• Experience managing COPD, asthma and diabetes
