Hospital Social Worker

The Hospital Integrated Discharge team consist of Qualified Social Workers, Social Care officers, Qualified Nurses and Therapists who all work closely with the multi-disciplinary teams using a HOME FIRST Approach across the wards and wider community.

The team is currently under new management and reconstruction bringing a dedicated and a can-do attitude to the service we provide. We are looking to recruit passionate and focused Social Workers to come and join our fantastic team of staff in providing a social care service within the Integrated Discharge Team across 2 sites at Fairfield General Hospital & North Manchester General Hospital (from time to time you may be required to travel to other hospital sites).

This will include working on a 7-day service rota basis covering weekends. (8.45 - 17.00)

The Integrated Discharge Team supports the wards to facilitate safe and timely discharges of patients from the hospital. To achieve this, they use a wide variety of personalised support, assist people to promote their independence, support people with complex needs and often changed situations to ensure they have the maximum level of choice and control available to them and promotes the principles of The Care Act and Mental Capacity Act.

We focus on how people can be supported by the strengths they have, their network of support from family, friends and their local communities. Where care and support is required, we work to ensure that this reflects the individual’s assessed needs and strengths, while promoting their independence, choice and control.

You will be expected to undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act. To undertake duties expected of Social Workers in line with the Professional Capabilities Framework and in accordance with the specific job description attached.
