Role Overview

An exciting opportunity for Community Nurses has arisen in Bury Town Integrated Neighbourhood Team. If you love meeting new people and working with a range of specialities the INTs are the place for you! The successful applicant/s will join a friendly team of community nurses, community matrons, therapists, INT coordinators and local administrators, supported fully by a Team Manager, and Localities Lead.

You will be providing nursing services to patients in the community and will have the chance to work in an autonomous manner with the support of the wider multi-disciplinary team.

Within the INT localities you will be using a wide range of community nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting. Experience of palliative care, wound management and catheter management would be an advantage. This will be the perfect opportunity if you would like a role where you can use a wide range of nursing skills, or learn new skills specific to providing clinical care to people in their own homes.

Key Responsibilities
  • Assess, plan and deliver healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
  • Work within the integrated team to facilitate early discharge from hospital.
  • Work within the integrated team to prevent unnecessary admission to hospital.
  • Work with all health care professionals, and statutory/non‑statutory agencies to provide a seamless, integrated service to our service users.
  • Communicate with patients, their relatives, carers and colleagues and use communication techniques to motivate, negotiate or persuade internal or external stakeholders.
  • Show empathy and understanding in verbal and non‑verbal communications.
  • Actively contribute to multidisciplinary team meetings and other meetings.
  • Support all team members within the integrated team, respecting their roles and contributions.
  • Be responsible for safe use and maintenance of equipment and supplies, be aware of budgetary constraints and work within these.
  • Be accountable for own professional actions as determined by the Nursing and Midwifery Council code.
  • Comment on draft policies.
  • Ensure staff are competent to undertake delegated work and take part in clinical supervision as per Trust Policy.
  • Ensure adherence to Trust policies, procedures and behaviour framework.
Skills and Qualifications
  • Strong assessment skills and confidence in providing safe, effective treatments in the community.
  • Experience or interest in palliative care, wound management and catheter management.
  • Ability to work independently and within a close‑knit multidisciplinary team.
  • Good communication skills, able to motivate, negotiate and persuade stakeholders.

You will have the privilege of working with patients in their own home environment to improve their health, alongside teams such as Virtual Ward and our Early Intervention Team (EIT). The role suits both newly qualified as well as experienced nurses and you will be able to develop your role in accordance with areas of clinical interest and to progress in your career, with expert training and education available to support this.

We operate a 7-day service with core hours 8am to 6pm. Access to your own vehicle is required and travel across the locality is part of daily working.
