Huisartsenpraktijk Vogelaar is op zoek naar een ervaren POH-somatiek. Het betreft een kleine, maar groeiende praktijk in Hilversum gevestigd binnen Medisch Centrum Dudokpark sinds 1 oktober 2025. De praktijk is sinds 1 januari 2026 gestart met ketenzorg Diabetes Mellitus.
- Als POH-S ben je verantwoordelijk voor de begeleiding van patiënten met o.a. diabetes mellitus.
- Je draait zelfstandig spreekuren.
- In de toekomst zal mogelijk ook gestart worden met ketenzorg CVRM.
- Gezien de opstartfase ben ik op zoek naar iemand die het leuk vindt om proactief mee te denken over de verdere ontwikkeling van ketenzorg binnen de praktijk.
- De praktijk zit samen met 2 andere praktijken binnen MC Dudokpark. We hebben gezamenlijke koffie- en lunchpauzes.
- Het aantal uren is 6 uur per week, met in de toekomst mogelijkheid tot uitbreiding. Werkdag(en) in overleg maar voorkeur voor maandag, donderdag of vrijdag.
- Arbeidsvoorwaarden zijn conform CAO Huisartsenzorg en in overleg. Je kunt gratis parkeren bij de praktijk.
