• Provide case management services to individuals who meet the definition of being developmentally disabled, as defined by the State of Montana.
• Assist clients in gaining access to medical, social, educational, vocational, and recreational services.
• Track caseload and billing requirements on a monthly, quarterly, and yearly basis.

• BS/BA in a social work related field and minimum of one year of direct work experience in the developmental disability field.
• Ability to develop and implement individual treatment plans, including behavioral, clinical, and mental health interventions.
• Understanding of the emotional and behavioral needs of assigned clients.
• Excellent interpersonal and communication skills; can work independently and as a team member, observe, report accurately, and follow instructions.
• Ability to facilitate meetings and work independently via a virtual office using a laptop and cell phone.

AWARE targeted case managers earn $20.00 - $22.19 per hour.
