Job overviewIn this role, you'll provide person-centred therapeutic support to older people experiencing mental health difficulties across community settings, care homes, and memory assessment services.
You'll carry out specialist assessments, reviews, and interventions for people with complex needs, including dementia, cognitive impairment, and other mental health conditions, making sure care is effective, evidence-based, and as least restrictive as possible.
You'll work flexibly across services—supporting patients, carers, and staff while also getting involved in service development and quality improvement.
You'll also help plan and oversee care for patients in both primary and secondary care, including those living in the community and in care homes.
Main duties of the jobThe main duties of this role involve working closely with older people and their families to provide compassionate, person-centred support, including having sensitive conversations around diagnosis and care needs in a reassuring and respectful way.
