Job Overview

An exciting opportunity has arisen for an experienced Band 7 specialist Nurse to lead our multidisciplinary Cardiac Rehabilitation Team in Caerphilly. We are seeking a highly motivated professional with a background in Cardiac Rehabilitation who is committed to developing and improving the service for patients across Gwent. You will need to be a good team player with the ability to be flexible, innovative and to work independently within the community setting.

The successful individual will be responsible for developing the service, while meeting changing needs in this challenging environment. Using your professional leadership and management skills you will be responsible for leading and supporting the wider multidisciplinary Cardiac Rehabilitation Team whilst also providing specialist input to patients referred to the service. You will be joining a friendly supportive team where professional guidance is readily available and continuous professional development is strongly encouraged.

Main

Duties
  • To lead the provision of core Cardiac Rehabilitation services within ABUHB, ensuring high‑quality, effective, evidence‑based and efficient care and taking responsibility for professional leadership and operational management.
  • To provide an integrated Cardiac Rehabilitation service, working closely with primary, secondary and tertiary care.
  • To work as an autonomous and independent practitioner within a multi‑disciplinary team, ensuring all Cardiac Rehabilitation patients receive the highest standards of care and support from referral to the service to discharge back to primary care.
  • Provide professional support, clinical expertise and appropriate education and training to all professionals involved in providing care to Cardiac Rehabilitation patients.
  • Lead and develop care in Cardiac Rehabilitation to meet the requirements of the British Association of Cardiac Prevention and Rehabilitation (BACPR) and NICE guidance, ensuring a continuous process of evaluation via the National Audit of Cardiac Rehabilitation (NACR) and striving for or maintaining certification via the National Certification Programme (NCP).
  • Undertake an active role in promoting and carrying out audit and research in conjunction with other health professionals, seeking to have work published.
