As a Support Practitioner, you’ll play a key role in helping people live their best lives. Every day, you’ll make a real difference by supporting daily routines, assisting people settle back home after a hospital stay, or responding to telecare alarms. It is a role where you help people stay connected, independent, and supported in their own homes.
Responsibilities- Support people with daily activities, including meals, medication, and personal care
- Build strong, trusting relationships with people and their families
- Work flexibly across different shift patterns, including days, evenings, and weekends
- Shift patterns vary from 7am – 11pm, with options of 7am – 3pm, 3pm – 11pm, or 11pm – 7am waking nights
- Respond to telecare alarms 24/7 and help people in emergencies such as falls
- Perform dynamic risk assessments and quick problem-solving using specialist equipment such as ELK cushions
- Assist with meal preparation, medication management, personal care and other practical support to maintain independence and quality of life
We are looking for people who are compassionate, reliable, and community‑minded. Full training is provided, and you’ll be supported to achieve an SVQ in Health and Social Care while you work.
Preferred but not essential: full driving licence, as many roles involve travel across rural communities.
Contracts & Benefits- Part‑time and full‑time contracts are available. Relief Support Practitioner roles also offer greater flexibility.
- Anti‑social hours enhancement up to £1,241 per annum for waking night shifts (11pm‑7am)
