Pembrokeshire County Council is seeking a Services Coordinator (Health & Wellbeing) to work with the Programme Manager on designing, delivering and evaluating targeted wellbeing initiatives across Pembrokeshire.
You will supervise a small team of Pathways Advisers and coordinate with GPs, healthcare professionals and local organisations to improve residents' health, reduce barriers to work, and signpost to suitable support.
