The Barnardos Achieving Change Together Plus Service will provide support for disabled young people (18-25) who live in Blaenau Gwent. This service will extend the support that is currently provided by the Achieving Change Together Service (0‑18).

• Provide opportunities for young people to gain skills and confidence to lead happy and fulfilled lives.
• Provide opportunities for young people to engage in their local communities, building their resilience and reducing isolation.
• Work in partnership with other services and organisations to provide a holistic plan of support for the family.
• Facilitate group work to promote skills development, confidence and independence.
• Support and enable disabled young people to access mainstream, specialist and community resources that meet their needs.
• Promote and increase inclusion and accessibility within communities.

As a project worker (PW1), your duties will include:

• Utilising effective communication skills to engage young people and families to build collaborative relationships.
• Supporting young people and families to identify strengths and prioritise support needs.
• Planning, facilitating and evaluating groupwork programmes and one‑to‑one support.
• Maintaining accurate records.
• Celebrating achievements / successes.
• Establishing, developing and maintaining productive working relationships with colleagues and other agencies.

You will have:

• Experience of working with young people and their families.
• Experience of undertaking assessments with young people to co‑produce holistic support plans.
• Experience of planning, facilitating and evaluating group programmes.
• Knowledge and experience of safeguarding.
• Ability to prioritise, organise, self‑motivate and work on own initiative.
• Experience of partnership working with a range of voluntary and statutory agencies.
