JOB TITLE: Licensed Clinical Social Worker

DEPARTMENT: Rural Health Clinics

REPORTS TO: Director of Clinic Operations

Summary for Position: LCSW to deliver high-quality care within our mental health program. If you're committed to evidence-based practices and excited to deliver culturally responsive care to patients of all ages.

• Perform diagnostic evaluations and interviews with patients
• Develop individualized treatment goals and recommendations based on patients' social, emotional, and psychological needs
• Maintain weekly clinical contact with patients
• Completion of documentation/progress notes after clinical contact within 48 hours in the EMR and notes delivered to the assigned representative at the facility
• Maintain working relationship and open communication with facility staff and direct care team
• Deliver weekly evidence-based therapy sessions
• Monitor client progress virtually through automated assessments and goal-based measures
• Collaborate with providers and fellow therapists for comprehensive care
• Maintain accurate, timely documentation through EMR

MINIMUM EDUCATION: Master’s degree or higher in counseling, psychology, social work, or related field. Must be fully licensed by the State Board to provide therapy independently and without supervision.

PREFERRED EDUCATION: Doctoral Degree

MINIMUM EXPERIENCE: 1 year of independent licensure

PREFERRED EXPERIENCE: 2+ years of recent experience delivering CBT and evidence-based therapy to youth and adults. Comfortable treating a diverse range of populations and mental health conditions. Experience working with higher-risk populations and/or adolescents.
