The Alex has developed a collaborative partnership with Onward – a property management company and developer in Calgary - to support low-income seniors in affordable housing communities, ensuring they can age in place, remain housed, and achieve better health and wellness. This partnership will address critical service gaps by combining Onward’s property management expertise with The Alex’s healthcare and social support services expertise.
Reporting to the Team Lead, the Case Manager provides individualized support to vulnerable seniors as they establish and maintain permanent housing within the Onward Home Murdoch Manor and King Tower mixed-market apartment buildings.
Operating within an Intensive Case Management model and using a strengths-based, client-centered approach, the Case Manager collaborates with program participants to develop personalized care plans with individualized goals. This role focuses on enhancing independent living skills, promoting social integration, facilitating access to healthcare and community services, and supporting eviction prevention strategies.

What You Will Do
Provide individualized, trauma-informed case management to residents, using a strengths-based and culturally competent approach.Conduct comprehensive intake assessments and develop individualized care plans, safety plans, and wellness contracts tailored to each resident’s unique needs and goals.Complete new intakes and regular reassessments to ensure timely support and appropriate service adjustments.Conduct psychosocial, needs, and risk assessments to identify potential barriers to housing stability, wellness, and safety.Connect residents with relevant community services and facilitate referrals to healthcare, mental health, substance use, financial, and social supports.Support residents in navigating complex systems such as healthcare, seniors’ benefits, legal services, and income support programs (e.g., AISH, CPP, OAS).Promote and support independent living skills, including meal preparation, medication adherence, financial literacy, personal hygiene, and housekeeping.Coordinate care transitions including hospital discharges, entry to higher levels of care, or home care support.Identify and mitigate risk factors that could lead to eviction, collaborating with property management, support workers, and residents to sustain tenancies.Provide crisis intervention and de-escalation support, referring to emergency services or mental health professionals as appropriate.Facilitate social, cultural, and recreational programming to reduce isolation and promote community connectedness.Advocate for residents’ rights, needs, and well-being, including support with appeals, legal navigation, and housing-related issues.Assist with transportation and accompany residents to essential appointments when necessary.Participate in multidisciplinary care planning and case conferencing with partner agencies, including Onward and The Alex Seniors Health Clinic.Collaborate with housing staff, community organizations, healthcare providers, and family members to ensure coordinated care.Maintain thorough documentation, complete regular case notes, assessments, and contribute to evaluation and data tracking systems.Participate in monthly partnership meetings…
