Working collaboratively within the Emergency Department Social Work team and the interdisciplinary care team, the incumbent will provide a full range of social work services, including psychosocial assessment, crisis intervention, patient and family support, discharge planning, resource navigation, advocacy and coordination of community services. The role will support timely assessment and intervention for patients presenting to the Emergency Department, including collaboration with triage, identification of diversion opportunities and addressing psychosocial barriers that may contribute to hospital admission or delays in disposition.
The incumbent will facilitate access to appropriate community and health system resources to support safe and effective care transitions and optimise patient flow throughout the Emergency Department and hospital.
Responsibilities As a Social Worker II you will be a member of an inter‑disciplinary team and provide a range of social work services to patients and their families. Services may include:
Psychosocial assessment
Resource‑based interventions
Individual, group and family treatment
Risk screening
Counselling
Case management and coordination
Transition or discharge planning
Prevention, promotion and public awareness activities
Required Qualifications Completion of a bachelor’s degree in Social Work.
Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Minimum six months experience in an Emergency Department setting, with the last two years in the role.
Minimum one year experience in acute care in the last three years.
Compensation Minimum Salary: $42.14 per hour.
Maximum Salary: $56.02 per hour.
Work Schedule FTE: 1.00. Hours per
Shift: 7.75. Length of Shift in Weeks:
4. Shifts per Cycle: 20. Shift Pattern:
Days, Evenings, Nights.
