Your role will be vital in enhancing residents' quality of life within a caring team framework.
This position means working collaboratively as part of a dedicated care team, supporting residents' daily needs and observing changes in their health status. Under the guidance of a Licensed Practical Nurse, you will play a crucial role in the development and execution of individualized care plans, ensuring each resident receives appropriate support and attention.
Key Responsibilities:
• Deliver resident care within your full scope of practice
• Monitor residents and report any noticeable changes
• Assist in creating and updating Nursing Care Plans
• Help residents with mobility and participation in community activities
• Report any safety concerns immediately
Requirements:
• Completion of a Health Care Aide program or equivalent
• Experience in community health services preferred
• Excellent communication and time management abilities
• Basic Cardiac Life Support certification is recommended
• Capable of performing physical demands of the role
