You're essential in monitoring changes and participating in nursing care planning to ensure that all residents achieve holistic well-being in a supportive environment.
Key Responsibilities:
• Provide resident-focused care within your practice scope
• Monitor residents and report any changes to nursing staff
• Participate in Nursing Care Plan development and implementation
• Support residents in community mobility and activities
• Report unsafe practices that may risk resident safety
Requirements:
• Health Care Aide program graduate or equivalent certification
• Experience in community health services for seniors
• Effective communication and customer service skills
• Proven organizational and time management abilities
• Current Vulnerable Sector Check required within six months
