Clinical Care Specialist; LPN

Granddaughters Home Care is seeking a Licensed Practical Nurse (LPN) with advanced clinical experience and leadership capabilities to join our team as Clinical Care Specialist
. In this role, you will provide clinical oversight, support new and existing care staff, ensure quality standards are met across service lines, and participate in strategic care planning. You will play a key role in leading clinical initiatives, supporting training and mentorship. This position offers opportunities for advancement and professional growth for the right candidate.

• Provide clinical leadership, mentorship, and ongoing support to frontline care staff.
• Conduct client assessments and contribute to the development and oversight of individualized care plans.
• Lead onboarding, training, and skills development for caregivers (including personal care and advanced foot care).
• Act as the primary clinical escalation point for complex care situations and incidents.
• Provide direct clinical support as needed in the community, particularly for complex cases or first visits.
• Serve as a clinical escalation point for care teams and collaborate with the CEO and Care Experience Manager on incident response and care concerns.
• Support Occupational Health & Safety (OH&S) initiatives and compliance.
• Maintain clear and timely documentation in Alayacare.
• Contribute to program development and continuous improvement of clinical practices and service offerings.

• Active and unrestricted LPN license in Alberta.
• Advanced Foot Care Certification (FCN) is considered an asset.
• Minimum (2) years' experience in a clinical leadership, supervisory, or advanced practice role.
• Minimum two (2) years of experience providing Home Care and clinical support to seniors living with dementia and other cognitive impairments.
• Strong mentoring, coaching, and interpersonal communication skills.
• Experience with client assessments, care planning, and clinical documentation.
• Knowledge of community home care best practices and healthcare regulations.
• High attention to detail, sound clinical judgment, and the ability to thrive in a fast-paced, flexible environment.
