This full-time role requires you to engage with clients at the Addiction Medicine Clinic while providing case management and connecting community members to health services. Your responsibilities will encompass assessments, harm reduction support, and participation in health education initiatives.
Key Responsibilities:
• Conduct assessments for community members in need
• Facilitate addiction intake and wellness recovery programs
• Provide health education and prevention programming
• Conduct home visits using the Mobile Clinic Van
• Support Contact Tracing efforts for STBBIs
Requirements:
• Minimum Diploma in Health or Social Services
• Two years of work-related experience in Mental Health
• Valid Ontario Driver’s License and own vehicle
• CPR and First Aid certifications
• Understanding of Anishnaawbe culture and language
