Reporting to the Health Promotion & Community Support Manager, the Community Health Worker will play a key role in improving access to health services and wraparound support by conducting client-centered intake and assessments, facilitating group programs and holistic lifestyle plans, coordinating referrals, and managing data collection and reporting. The successful candidate will also collaborate with interdisciplinary teams and community partners to design, implement, and evaluate strategies that address the social determinants of health of this priority population and promote equitable health outcomes targeted towards healthy aging.

Primary ResponsibilitiesDeliver health promotion and education activities.
Facilitate community development and outreach.
Build partnerships with local organizations and networks.
Support communications and program awareness.
- Assist with data collection, reporting, and program coordination.
Post-secondary education in a related field.
3+ years’ experience in community health and/or outreach and engagement.
Knowledge of health equity and anti-oppressive practices.
Strong communication and facilitation skills.
Familiarity with Etobicoke communities is an asset.
