Key Responsibilities:
• Conduct psychosocial assessments to guide treatment decisions
• Facilitate discharge planning and care options for patients
• Provide teaching and resources to support patient needs
• Advocate effectively for patient and family concerns
• Lead initiatives for bereavement care within the program
Requirements:
• Master’s degree in Social Work from an accredited institution
• Current registration with the Ontario College of Social Workers
• Strong clinical skills in counseling and therapy
• Proven ability to manage complex cases in healthcare
• Experience in navigating community resources
