Registered Dietitian Foothills PCN

Join Calgary Foothills Primary Care Network as a Registered Dietitian focused on nutrition for chronic health conditions. Collaborate with a multidisciplinary team to deliver patient-centered care in clinical settings.

This role involves working with physicians and community partners to provide nutrition education and care strategies during individual and group appointments. You'll create tailored care plans based on clinical assessments and act as a vital resource for both patients and healthcare providers. Previous experience in primary care and group facilitation will enhance your contributions.

Key Responsibilities:

• Provide nutrition care through individual and group sessions

• Develop patient-centered nutrition care plans and assessments

• Educate patients and teamwork through evidence-based information

• Participate in nutrition program planning and quality improvement

• Maintain data for service indicators and evaluations

Requirements:

• Bachelor of Science in Foods and Nutrition

• Completion of Dietetic internship

• Active registration with the College of Dietitians of Alberta

• Minimum three years of experience preferred

• Flexible with working hours, including evenings and weekends
