Position: Social Worker for Seniors at AgeCare
Make a meaningful impact at Age Care as a Social Worker specializing in senior care. Facilitate holistic support, including counseling and assessment, for residents and their families in long-term care settings.

As a Social Worker reporting to the Director of Care, you will be an integral part of an interdisciplinary team.

Your role includes conducting comprehensive psychosocial assessments, establishing rapport, and providing tailored interventions for our residents. Engage in consultation, adjustment support, and referral services to ensure the well-being of both residents and families.

Key Responsibilities:

• Conduct psychosocial assessments for newly admitted residents

• Establish rapport with residents and families

• Provide counseling and educational support

• Facilitate financial and legal assistance as needed

• Utilize various therapeutic techniques for effective communication

Requirements:

• Registered Social Worker under the Health Professions Act

• Bachelor of Social Work from an accredited institution

• Experience working with geriatric populations

• Ability to work in a multi-disciplinary team

• Willing to obtain a Vulnerable Sector Check
