Make a difference in healthcare as an Intake Coordinator at Mosaic PCN's Mental Health Program. This casual role facilitates patient intake processes and coordinates services to support mental health needs.
As an Intake Coordinator, you will be part of a multidisciplinary team dedicated to enhancing patient experiences in community health. Your responsibilities include conducting clinical assessments, documenting care, and coordinating referrals to resources. You'll utilize your interpersonal skills to guide patients through the intake process while maintaining compliance with regulations.
Key Responsibilities:
• Conduct clinical intake appointments for mental health referrals
• Perform psychosocial and risk assessments under supervision
• Coordinate crisis support resources as needed
• Schedule appointments with mental health therapists
• Facilitate referrals to community mental health services
Requirements:
• Social Work Diploma or related bachelor’s degree
• Current registration with professional association
• Minimum 2 years of direct clinical experience
• Skills in crisis intervention and risk management
• Strong communication skills and teamwork capabilities
