As part of the Intensive Case Management outreach team, your expertise is vital in managing health stability for individuals with complex needs. This position leverages your nursing skills to perform assessments, educate participants, and enhance overall community integration. A collaborative approach with case managers is essential to mitigate health and housing challenges.
Key Responsibilities:
• Provide trauma-informed care and health education
• Identify risks impacting health and housing stability
• Support participant navigation to health services
• Track nursing activities and maintain documentation
• Work collaboratively with an interdisciplinary team
Requirements:
• Active registration as a Nurse with CRNA
• 3-5 years' relevant nursing experience
• Experience with mental health and addiction care
• Class 5 driver’s license and clean driving record
• Criminal record clearance needed
