This role requires a skilled Registered Nurse to deliver comprehensive nursing services with an emphasis on problem-solving and patient advocacy. Focus on case coordination, holistic patient care, and health promotion in a rapidly changing environment. You will help clients with a range of health concerns, including wound care and palliative needs.

Key Responsibilities:

• Provide quality assessments and nursing care

• Collaborate with clients and families for effective care

• Manage home palliative and wound care therapies

• Uphold patient safety and health standards

• Report directly to the Home Care Manager

Requirements:

• Completion of accredited nursing education

• Current registration or eligibility with CRNA

• Certification in Basic Cardiac Life Support

• Minimum 2 years in Continuing Care/Home Care

• Vehicle and driver's license required
